To enroll in the ABCD program your child must be under 6 years of age, and receive medical assistance coupons.

You may enroll you child at one of the sites listed in the Orientation Schedule or ask your dentist if he is an ABCD certified dentist who enrolls at the dental office. You will need to show your child’s medical coupon at this time.

Your ABCD identification card will be given to you at the time of enrollment.
